Re: 2006 CAWP/H2AP Safety Banquet
The Safety Banquet will recognize Accident-Free Supervisors and Companies for their safe work during the 2006construction season. Accident-Free Supervisors are crew foremen, general foremen, and superintendents who have supervised a minimum of 5,000 hours with zero Lost Workday Accidents in 2005. Companies with zero Lost Workday Accidents in 2005, and companies with Lost Workday Incidence Rates below the national average for 2005, will also be recognized at the Safety Banquet.

- AGENDA: 5:00 – 6:00 PM Registration, Happy Hour & Vendor Displays
6:00 – 7:00 PM Award Presentations & Buffet Dinner
7:30 PM Races Begin!
- AWARDS: Supervisors - 2006 coin set & gift (Accident-Free Supervisors must be registered to attend
the Banquet to receive their award)
Company Awards – Engraved plaques & framed certificates
Personnel: Complete the Registration Form for Accident-Free Supervisors and Management attendees.
Companies: Complete the Company Award Eligibility Form to determine if Company is eligible for an award based on Lost Workday Incidence Rate. Only include heavy, highway, railroad and utility work performed in the 33 counties of western Pennsylvania in your calculations.

The forms must be received no later than Wednesday, May 17, 2006.
- COST: $35.00 per person (includes food, beverages, and admission to the races)
Please make checks payable to H2AP and submit when registering.
No refunds will be issued for cancellations received after Friday, May 26, 2006.
