Banquet Recognizes Safe Supervisors & Companies
The CAWP/H2AP Safety Banquet will recognize Accident-Free Supervisors and Companies for their safe work during the 2009 construction season. Accident-Free Supervisors are
crew foremen, general foremen, and superintendents who have supervised a minimum of 5,000 hours with zero Lost Workday Accidents in 2009. Companies with zero Lost Workday Accidents in 2009, and companies with Lost Workday Incidence Rates below the national average for 2009, will also be recognized at the Safety Banquet.

LOCATION: Lernerville Speedway, 278 N. Pike Road, Rt. 356, Sarver, Pennsylvania 16055
AGENDA: 5:00 – 6:00 PM Registration, Happy Hour & Vendor Displays
6:00 – 7:00 PM Award Presentations & Buffet Dinner
7:30 PM Races Begin!
AWARDS: Supervisors - 2009 U.S. coin set & gift (Accident-Free Supervisors must be registered to attend the Banquet to receive their award)
Company Awards – Engraved plaques & framed certificates
DRESS: Casual
REGISTER:
Personnel: Complete and submit the
Registration Form for Accident-Free Supervisors and Management/Office Personnel attendees no later than
Wednesday, May 26th.
Companies: Complete and submit the
Company Award Eligibility Form to determine if you are eligible for an award based on Lost Workday Incidence Rate, no later than
Friday, May 21st. Only include heavy, highway, railroad and utility work performed in the 33 counties of western Pennsylvania in your calculations.

No refunds will be issued for cancellations received after
